2026 forms are required to be submitted in hard copy format.
Forms can be returned to the band director (including middle schools), to the RED BOX at Lakeland or during practice to the BLUE BOX in the Milford Band Room

Huron Valley Schools Criminal Record Check
This form must be completed AFTER July 1 to count for the 2026-2027 season. Must be completed before July 15 if you are planning to chaperone at Band Camp.
