Marching Band Forms

Please read through all the information below and note the due dates of the different forms.  Here is a quick overview of when they are due:

Forms 1-7  – DUE:  05/15/2024
Remaining Forms:  DUE:  07/01/2024

Couple of notes about the forms: 

  • Parents/Guardians and Students, please work together on these so that only one form is fill out for each student.
  • Please start to think about scheduling your physicals and plan to do them in June, this helps as it covers the whole season, which runs from July – May.  Student WILL NOT be able to attend camp in August if we do not have a physical by then.  Huron Valley Schools does offer some low cost physicals (typically it’s been $20) nights during June at both High Schools.  Once we get information on when that will be we will send out that information.
  • The Band Camp Central Registry form, which is required for any parent to help at Band Camp requires a copy of the State ID or Drivers License, please not selfies or other photos as it will be rejected.   We encourage all parents to fill out if you think you would like to help at band camp in any form.
  • The Ford Field Waiver form is for ANYONE who would/will step out on the football field to help, parents or students.  If you have multiple kids in the program, you only need to fill out ONE form. But parents, though, if mom and dad or grandma, or guardians will be helping, we do need a form for EACH adult on the field. You do NOT need to list all students on the form again, just the additional adult forms.

All of the forms listed below are forms that we need for students and parents for this fall.  Please note below the purpose of each, and the dates due/method of submission.

Forms Due by May 15, 2024 – This year all of these will be submitted via digital “JOT” form submission after you register for band and receive the email with links.

1 – 2024 Huron Valley Marching Band Official Contract –  https://form.jotform.com/241153620649151
This form is your official contract to participate in the marching band this year due with your non-refundable deposit of $250. (included in this packet is a form for paying online/via credit card for all payments/volunteer opt-out fees).

Specific to this form – Please make sure you turn in 1 form for each student (don’t list multiple students on one form)


2 – 2024 Huron Valley Marching Band Student Attendance Contract https://form.jotform.com/241153634604147
This form is your agreement as a student to participate fully in all band practices/events/performances.  This form is due with your Official Contract and Deposit. 
(Please note there is a separate page to list out any per-arranged absences that we ask for you to submit on/before July 1, 2024).

Specific to this form:  Please make sure you turn in 1 form for each student (don’t list multiple students on one form)

This form allows your student to travel with us for all band activities.  It is good from the first day of band camp thru June 30, 2025.

4 – Chaperone Excursion Form https://form.jotform.com/241153248716152
This form allows parents to travel with us for all band activities.  It is good from the first day of band camp thru June 30, 2025.  Please complete a form for each parent.  If you don’t use it, no worries.  If you don’t submit one, you can’t travel with us.  Please complete the form to be covered just in case.

This form is REQUIRED of all parent volunteers (not just chaperones).  Any parents coming to any event, in any capacity, to help/volunteer are required to complete this form.  Please complete a form for each parent.  If you don’t use it, no worries.  If you don’t submit one, you can’t travel with us or volunteer for many events.  Please complete the form to be covered just in case. This is a link to the online district form that is required by Huron Valley Schools.

Specific to this form:  Please make sure you turn in 1 form for each parent/adult – Also please use 07/01/2024 as the Needed By Date


6 – State of Michigan/Band Camp Central Registry Clearance Formhttps://form.jotform.com/241152773754156
This form is REQUIRED of all parent volunteers (not just chaperones) who attend band camp, even for a few hours.  We ask every parent to complete this. If we are in an emergency and need you to help, and you don’t have a form submitted, it is not a quick process to get approved and we would not be able to have you come to camp.  Please complete a form for each parent.  If you don’t use it, no worries.  If you don’t submit one, you can’t come to camp at all with us.  Please complete the form to be covered just in case.  

Specific to this form:


  • PLEASE MAKE SURE YOU UPLOAD A PHOTO/COPY OF YOUR DRIVER’S LICENSE OR STATE ISSUED ID.  IN ADDITION, please make sure the address on your form matches your address on your ID.  
  • SOCIAL SECURITY NUMBER IS REQUIRED BY THE STATE AND IS NOT OPTIONAL.
  • Please make sure you turn in 1 form for each parent/adult (don’t list multiple names on one form)

7 – Ford Field Form (students/adults/parents) –  https://form.jotform.com/241153715165149
This form is for ALL students/adults/parents in the marching band.   EVERY STUDENT MUST HAVE A FORM ON FILE OR THEY CANNOT PARTICIPATE AT STATE FINALS.  Parents – This form is for all parents who will be attending State Championships at Ford Field. Parents, please complete the form as well (it doesn’t commit you to going to Ford Field, but if you decide to travel as a roadie/equipment person and end up coming on to the field with us at State Finals, YOU MUST HAVE A FORM SUBMITTED).  

Specific to this form:  Please make sure you turn in 1 form for each adult (separate forms), and include your student(s) on one of the forms being submitted…all students can be listed on one form, but each adult must submit their own form.

Forms Due by July 1, 2024 – submit via digital “JOT” form Submission:

1 – Event/Performance Absence Reporthttps://form.jotform.com/241153668373157
This form lets us know for planning any dates that a student will be absent.  Please check the entire season calendar and notify us of any conflicts (it is critical we know, especially on performance dates, but for all dates in general).  Mail this form as indicated on the sheet.  This form is due by July 1, 2024.


Forms Due by July 1, 2024 – This year ONLY THESE FORMS NEED TO BE SUBMITTED HARD COPY PLEASE.  EITHER MAIL THEM OR TURN THEM IN DIRECTLY TO THE RED BOX.

This form allows you to purchase the items students need to buy regarding band uniforms/apparel.   This form is due by July 1, 2024 with payment.  (The form will vary based on your student).

Specific to this form:

  • Do not use this form to order parent jackets or jackets for returning students (WE WILL HAVE A SEPARATE ORDER FOR THIS LATER IN THE SUMMER).
  • THIS FORM IS NOT SUBMITTED ONLINE VIA JOT/DIGITAL. Print the form and complete it and turn in by July 1.  You can turn it in to the red box in either band room or you can mail it to:

This form is essentially our version of a physical form.  If you have a physical done on a separate form (such as the HVS Athletics Physical Form or a form from your doctor’s office), please attach the original to our form.  The physical form must be completed and in our hands before we go to band camp, no exceptions.  If your insurance only covers 1 physical a year, and you have a current one that is still valid for a portion of the season, we simply need a copy of that physical on file and attached to our form.  The physical form must be replaced by a new one during the season prior to the one you submitted expiring!  It is your responsibility to make sure this is done.  This form is due July 1, 2024.

Specific to this form:

  • Please make sure you turn in 1 form for each student (don’t list multiple students on one form)
  • Make sure the physical is completed, signed by physician, and includes their address.  
  • Make sure all medical information is accurate and up to date (including medications)
  • THIS FORM IS NOT SUBMITTED ONLINE VIA JOT/DIGITAL. Print the form and complete it and turn in by July 1.  You can turn it in to the red box in either band room or you can mail it to:

4 – Parent/Guardian/Student Approval Form (MUST BE NOTARIZED)https://huronvalleymarchingband.com/wp-content/uploads/2024/04/2024-Parent-Approval-Form.pdf
This form is our Emergency contact form, and parent permission form for out of state trips, medical attention needed on trips, etc.  This form has many parts to it, so please check over everything needed.  The form MUST have a notary stamp but we also need you to complete the Parent and Student Approval Form portion and all the information/documents required.  This form is due July 1, 2024 as well.

Specific to this form:

  • Please make sure you turn in 1 form for each student (don’t list multiple students on one form)
  • Parents – please make sure all information is filled out, carefully checking off OTC medications on the parent approval portion.
  • Make sure it is notarized by a notary public.
  • Make sure the student signs the form under the Student Agreement
  • Attach a copy of your insurance card (both sides) for coverage
  • THIS FORM IS NOT SUBMITTED ONLINE VIA JOT/DIGITAL. Print the form and complete it and turn in by July 1.  You can turn it in to the red box in either band room or you can mail it to:
